Quién decide qué es ciencia: poder profesional y salud. Paradigmas, poder y resistencia al cambio en los sistemas sanitarios

Este artículo analiza críticamente cómo los paradigmas científicos dominantes en los sistemas sanitarios definen qué conocimientos se consideran legítimos y quién ostenta la autoridad para producirlos. Desde el marco teórico de los cambios de paradigma, se argumenta que el modelo biomédico positivista ha evolucionado hacia una posición dogmática que limita la integración de enfoques centrados en el cuidado, la participación y la comunidad. Esta hegemonía contribuye a la fragmentación, la deshumanización y la escasa capacidad de respuesta de los sistemas sanitarios ante necesidades complejas. El trabajo sostiene que la resistencia al cambio paradigmático no responde a la falta de evidencia, sino a dinámicas de poder profesional, y defiende la ampliación del modelo científico como una exigencia de honestidad científica y responsabilidad social.
