Inmunización contra la tuberculosis: a 100 años de la BCG

Resumen

Cada día  más de 30,000 enferman de tuberculosis en el mundo. Esta enfermedad, que muchos creen del pasado, sigue cobrando 1,2 millones de vidas al año en pleno siglo XXI. La única vacuna disponible, la BCG, tiene 100 años de antigüedad y, aunque protege bien a los niños pequeños, falla sistemáticamente en adolescentes y adultos. ¿Por qué hemos esperado un siglo por nuevas vacunas? El reporte mundial de la tuberculosis  2025 de la Organización Mundial de la Salud (OMS) muestra un panorama inédito: 18 vacunas candidatas están siendo probadas en humanos en este momento. Algunas buscan prevenir la infección, otras evitar que resurja en personas ya curadas, e incluso hay propuestas para mejorar los tratamientos actuales. Este artículo explora por qué ha sido tan difícil desarrollar nuevas vacunas contra tuberculosis: Mycobacterium tuberculosis es una bacteria extraordinariamente compleja que desafía nuestro sistema inmune de formas que aún no comprendemos completamente; los modelos animales han fallado repetidamente en predecir qué funciona en humanos; y probar una vacuna requiere estudios con decenas de miles de personas durante años. Entre los candidatos más prometedores destacan M72/AS01E, que según estudios recientes, logró proteger a la mitad de las personas vacunadas; MTBVAC, diseñada directamente con la bacteria con muchos más componentes que BCG; y vacunas de ARN mensajero, la misma tecnología que revolucionó la lucha contra COVID-19. Después de un siglo de espera, varias vacunas con estrategias diferentes podrían finalmente cambiar la historia de la tuberculosis.
